L’allergie aux proteines du lait de vache chez le nourrisson (A propos de 20 cas)

Cow’s milk allergy is the first allergy to appears in children, its prevalence is high, affecting 2 to 3% of infants in unselected cohorts. Cow milk protein allergy is characterized by a wide range of symptoms and signs affecting the gastro-intestinal tract, the respiratory system and the skin. A better definition, a stricter application of diagnostic criteria and critical evaluation of certain immunologic correlates can contribute to a better understanding and preventive treatment of this entity. In this regard, and after a recall on the generalities of the disease, we analyzed the data from this disease in 20 patients and we try to identify the epidemiological, clinical, therapeutic and outcome. Our work consists of a retrospective study 20 cases of cow’s milk allergy, recruited to the pediatric department of the CHU Hassan II of Fez, during period going of 1er January 2006 to 31December 2009. The frequency of ALPV is estimated at 0.6% of patients hospitalized during the same period. Average age of onset is 2 months, the extremes of age between 2 and 19 months. The deadline for consultation is often late. The disease affects both sexes, with a slight female predominance. Concept of atopy was noted in 10% of cases. Diarrhea is the predominant symptom in 85%, with 80% vomiting. Symptoms extra digestive respiratory and skin are found in 20% of cases. All our patients were put under exclusion regime with replacement of protein hydrolysates, until the age of 1 to 2 years, the period when, usually, immunotolerance is acquired. The initial evolution was favorable for all patients on exclusion regime. The tolerance to cow’s milk should be controlled by a test reintroduction, before allowing the resumption of a normal diet, in our series, no cases have benefited from this test because most of our patients were lost of view and other hand, the time needed for the reintroduction is not yet gone to those who have regular monitoring. Cow’s milk allergy is one of the most common allergy in infants, it must strengthen the promotion of breastfeeding and delaying the introduction of artificial milk
