La fièvre boutonneuse méditerranéenne :profil épidemio-clinique, thérapeutique et évolutif (A propos de 66 cas) expérience du service de dermatologie de l’hôpital militaire Moulay Ismail-Meknès

The mediterranean spotted fever is a rickettsial eruption, caused by Rickettsia conorii, transmitted by the bite of the brown dog tick Rhipicephalus Sanguineus.In Morocco, the mediterranean spotted fever is known since the twentieth century, it is a real public health problem.This disease is not contagious and does not need isolation. Its declaration is required.Currently, the epidemiology of mediterranean spotted fever in Morocco, as in North Africa, is not well known or even unknown.All cases of mediterranean spotted fever recruited from Dermatology service of Meknes Military Hospital between January 2006 and December 2014 were analyzed noting the age, sex, profession, concept of animals in the entourage, active lesions and disease duration (between emerging and medical consultation). During this period, 66 cases of mediterranean spotted fever were recruited.A male predominance was observed (89%), with an age range from 20 years to 30 years.The disease has a seasonal nature, it predominates in summer. Indeed, 95% of cases occur between May and October.Patients are from urban and suburban areas, and we did not notice a difference between these two areas.After an average of six days of incubation, the majority of patients have a classic form of the disease with fever, rash and often eschars.In our serie, 9 % of cases were severe forms with complications and multivisceral malignant forms. Risk factors were identified in those cases: male gender, advanced age, chronic smoking, diabetes, high blood pressure, taking inappropriate antibiotic and anti-inflammatory drugs, and the delay between the onset of the disease and the therapeutic management.In our retrospective study, no bacteriological study has been done, the diagnosis was based on anamnestic and clinical arguments.Doxycycline was the treatment of choice with a good evolution in the majority of cases.Major efforts are still needed to focus on preventive actions and the development of a vaccine
