COÛT DE LA PRISE EN CHARGE DU KYSTE HYDATIQUE DU FOIE COMPLIQUE AU CHU HASSAN II DE FES

Hydatidosis is a parasitic disease considered benign but can become dangerous because of its complications. It is a real public health problem in the world by its frequency, morbidity, potential mortality as well as its socioeconomic impact. The main objective of this work is to assess the direct medical cost for the management of the complicated form of hydatid cyst of the liver at the University Hospital Hassan II of Fes, to compare it to the cost of the simple form and to study the epidemiological aspects of the population of the region. It is a medico-economic retrospective study carried out between January 2009 and June 2012 from data of patients who were treated for hydatid cyst of the liver in the departments of surgery A and B at the University Hospital Hassan II of Fes. The results of the medical study show that the hydatid cyst of the liver affects young adults with an average age of 39.67 years and a clear female predominance. The majorities of patients are low socioeconomic level and come from rural areas. Complications represent 30 % of cases. The most frequent complications are rupture in the biliary tract and infection that were found respectively in 71,19 % and 40,68 % of complicated form. 26,15 % of patients with a complicated hydatid cyst of the liver were hospitalized in the resuscitation department. The average length of hospitalization were 24,35 days for the complicated form and 12,45 days for the simple form. The results of the economic study shows that the direct medical cost for the management of hydatid cyst of the liver is 19,925,84 MAD for the complicated form, and 6.026,02 MAD for the simple form. For the complicated form, an increase of all items of expenditure was recorded especially for hospitalization, drugs and sphinctérotomy. The admission by the circuit of departments of emergencyresuscitation-surgery represents 23,08 % and it is associated with the higher cost of the management that is 32.173,09 MAD.. University Hospital Hassan II of Fes participates annually with 580.951,88 MAD which represents 85,96 % of the total direct medical cost for the management of surgical form of hydatid cyst of the liver. The overall medical cost is 2,365,464.21 MAD for the entire duration of our study. These results show the significant economic impact engendered by hydatidosis in hospitals, which involves developing an effective and efficient national strategy to control this disease that has been eradicated in many countries through control programs based essentially on prevention and proper medical care. The respect of the filiation of providing care can accelerate the management of patients in the structures providing second level of care and thereby reduce the long waiting lists.
