LES NOUVEAU-NES DE MERES REFEREES (Aproposde112cas)

In spite of the efforts supplied by the ministry of health to fight against the maternal and infantile mortality, Morocco knows high neonatal deaths. Our objective is to study the reference of parturients in our context and its impact on the inborn: quantify prevalence of the inborn from referred mothers, specify the main reference causes, determine the duration of transfer, analyze the causes of neonatal deaths and finally set up an immediate Action plan. Material and Methods: It is a retrospective study about 112 cases of inborn from referred mothers and taked care in the department of neonatology and neonatal reanimation of UH (University HOSPITAL) HASSAN II of Fez during a period of one years from 1st January to 31 December, 2010. Results: In our context, the prevalence of the newborn of referred mothers is equal to 10,30 %. The main reference indications were in decreasing order: the preterm prelabour rupture of membranes PPROM (25 %), the preterm contractions PC (12 %), the preeclampsia (9, 5%), the fetal suffering (8, 5 %), the childbirth dystocique (8 %) and the bleeding (7 %). The rate of caesarian is 50 %. The main references from Sefrou (37 %) and Taounate (28 %). The durations of transfer were related to the distance traveled with an average of 113 min which remains a long duration in the emergency context. The inborn are mainly admitted in the service of neonatology and neonatal reanimation of UH (University HOSPITAL) HASSAN II of Fez for respiratory distress syndrom followed by the neonatal suffering, the prematurity then by the infection materno-fetale with rates of 26 %, 25 %, 24 % and 16 %, respectively. The evolution of the inborn was marked by an improvement and a favorable response to 75 cases, while 37 cases died after a multiorgan failure in 60 % of them. The precocity of supported inborn affect significantly the duration of hospitalization because of half (48 %) of the inborn hospitalized duration did not exceed 3 days.
