LA PLACE DE LA CUPULE ATLAS DANS LAPROTHÈSE TOTALE DE LAHANCHE CHEZ LE SUJET JEUNE (A propos de 80 implants)

This work is a retrospective study of 67 patients aged less than 50 years who has benefited from total hip arthroplasty « ATLAS » at the department of orthopedic (A) surgery of CHU Hassan II of Fez between january 2009 and january 201 5. The aim of our study is to show the value of « ATLAS » cup as successful surgical procedures in the treatment for long-term pain and in restoration of function for patients.In this study we include 67 patients (80 implants); they were 42 men and 25 women. The mean age at the time of surgery is 33,(range: 17 years and 50 years).All patients have been evaluated by clinical examination and radiography of hip and functional level was estimated by PMA and HARRIS scores. The different diagnoses that indicate total hip replacement were: 30 cases of inflammatory arthritis, primary arthritis in 7cases, 6 cases of post-traumatic osteoarthritis, 14 cases of fémoral head necrosis, 8cases of dysplasia, and 2cases of squeals of coxalgia.In this séries, 100% of patients were operated by general anesthesia. The Moore posterolateral approach was used in 100% of hips.Postoperative complications were: 1 case of preoperative hemodynamic instability, 1 case of early dislocation and 2 cases of edgings .
The PMA postoperative was excellent in 59% of case, good in 30% of case , medium in 8% . The HARRIS increased from 47,29 in pre-operative to 87,85 postoperative.According to this study we conclude that ATLAS cup is a successful prothesis to stop pain and to increase hip function
