Squamous cellcarcinomaarisingfromperinealfistulainpatientswithCrohn’sdisease(CD)isa rare entity,withfewreportedcasesintheliteraturemakingitsclinicalcharacterization troublesome. Theoutcomeispoorfollowingoperativetreatmentasthemalignancyofchronic perineal fistulainCrohn’sdiseaseisusuallyoverlookedandtardilydiagnosed.Wepresenta unique caseofa47-year-oldmanwitha20-yearhistoryofchronicperinealfistulathatwas diagnosed withextensiveperinealsquamouscellcarcinomaextendingdowntotherightthigh. Computer tomographyshowedlocallyadvanceddiseasewithinguinalandlungmetastasis. Treatment wasconsistedofdivertingcolostomyandpalliativecare.Thepatientdied3months later. Theclinicaldataandclinicopathologicalfeaturesofreportedcasesintheliteraturewere reviewed
