PRÉVENTION DE L’HYPOTENSION ARTERIELLE INDUITE PAR LA RACHIANESTHÉSIE AU COURS DE LA CÉSARIENNE PROGRAMMÉE CO-REMPLISSAGE PAR HEA VS SÉRUM SALÉ ISOTONIQUE

Introduction: Spinal anesthesia is the most commonly used method forn caesarean because of its simplicity and efficiency. However, its main inconvenience is the hemodynamic alteration on the parturient and the fetus.The aim of this study was to compare the efficacy of HES coloading compared to normal saline solution for prevention of hypotension during spinal anesthesia for elective caesarean section.
Patients and methods: it is a randomized prospective study, achieved in the obstetric operating room of Al Farabi Regional Hospital during a period of 7 months (From January 2014 to July 2014). Fifty patientswere recruited. They were randomly divided to two groups: Group I receiving saline solution Coloading in even days, and Group II receiving HEA colading in odd days.
Results: Patients of both groups had similar demographic and anthropometric data. They also had similar clinical history. Hypotension occurred in 34 patients in Group I and 8 patients in Group II (p = 0.002). Ephedrine consumption was significantly lower in Group II (P = 0.002). Nausea and headache incidence was higher in Group I (p = 0.02). Apgar scores were comparable between groups.
Conclusion: HES coload was more effective than normal saline solution to prevent hypotension following spinal anesthesia for elective cesarean section. HES coload reduced the incidence, the duration of hypotension, the need for ephedrine and the adverse maternal effects
