EVALUATION ALONG TERME D’UNE DEMARCHE EDUCATIVE DE PATIENTS ATTEINTS DE POLYARTHRITE RHUMATOIDE

Introduction:Rheumatoid arthritis (RA) is a chronic inflammatory rheumatism distorting and destructive. Medical treatment of rheumatoid arthritis is necessary but still insufficient for a comprehensive care. The multidisciplinary approach including education for disease, is a more appropriate therapeutic approach. The aim of our study was to evaluate the effects of an educational program on the development of rheumatoid arthritis (RA) after 2 years.
Materials and methods:We conducted a case-control study in Rheumatology CHU Hassan II of Fez over a period of 24 months. We included patients with rheumatoid arthritis that meet the criteria of the ACR 1987 or ACR / EULAR 2010 followed in the Rheumatology Department of CHU Hassan II of Fez. The educational program was held on a day with a briefing and educational workshops. The effects of the program were evaluated at D0 and at 1 and 2 years after in the educated group (EG) in the following variables: knowledge of RA (selfquestionnaire clinical case), activity disease (DAS28 VS), functional disability (HAQ) and quality of life (EMIR short). The results were analyzed at different times of the study in the group educated and then compared to day 0 and 2 years in the control group (CG). There have been no therapeutic change throughout the period of the study to better assess the effectiveness of the educational program.

Results:The knowledge of patients 2 years after the training program were significantly improved compared to baseline (p <0.0001) and were significantly better than in controls (p <0.0001). The disease activity was significantly lower in EG after 2 years (p <0.0001) compared with the CG (p <0.0001). HAQ has noted a significant variation after 2 years compared to baseline (p = 0.001) and compared to controls (p = 0.02). CONCLUSION:This work highlights that an educational program tailored to the patient's needs can lead to a sustainable improvement in the knowledge of the disease and may help control the activity of RA and its functional impact. These results support the development of education program for patients with chronic inflammatory joint disease
